This scenario would focus specifically on the populations that currently do not have equal access to the healthcare system or a less-than-average standard of care as in the same way as many middle class Americans do.  Who has limited access?  Why, what are the obstacles?  What are the factors such as  culture, cost, transport etc that limit access and the best standard of care?
